• Select your medication(s) using the search tool at the top of each page of this web site. Please be careful to select the strength and quantity that corresponds to your prescription.
• After each selection click on "Buy." When you have made all your selections, click on "Checkout."
• Complete all online forms as indicated:
  • Patient medical history and health questionnaire
  • Patient disclaimer
  • Billing information
• Print the medication order form and attach your prescription, then:
  • Fax to Canada Prescriptions Plus (toll-free) 1-877-979-7587, or
  • Mail to Suite #551  102-17750 #10 Hwy, Surrey, BC, CANADA V3S 1k4.
  • You may have your doctor fax or mail your prescription to CPP directly.

• Select your medication(s) using the search tool at the top of each page of this site. Please be careful to select the strength and quantity that corresponds to your prescription.
• Call toll-free 1-866-779-7587 (24 hrs/day, seven days/week). A CPP customer service representative will fax, mail or email you a copy of the patient order form.
• Fax the completed forms along with your prescription (toll free) to 1-877-979-7587, or Mail to Canada Prescriptions Plus Suite #551  102-17750 #10 Hwy, Surrey, BC, CANADA V3S 1k4.
• You may have your doctor fax or mail your prescription to CPP directly.
