• After each selection click on "Buy." When you have made all your selections, click on "Checkout."
• You do NOT have to fill out new patient profile and patient disclaimer forms.
• When prompted (in Step 3 under Rx Type), select “Refill.”

BY PHONE

• Select your medication(s) using the search tool at the top of each page of this site. Please be careful to select the strength and quantity that corresponds to your prescription.
• Call toll-free 1 866 779 7587 (24 hrs/day, seven days/week). A CPP customer service representative will take your order.
• Make sure to advise the customer service representative that your order is a refill.

BY FAX

• Click here to download and fill out the medication order form.
• You do NOT have to fill out new patient profile and patient disclaimer forms.
• Fax the completed medication order form to (toll free) 1-877-979-7587.
• Make sure that you indicate "Refill" in the appropriate box located on the form.
