Indications

Fosamax Plus D is a prescription medication that combines two active ingredients: alendronate and cholecalciferol (vitamin D3).

Alendronate is a bisphosphonate medicine that helps to slow down bone loss and increase bone density. It works by inhibiting the activity of cells that break down bone, thereby reducing the risk of fractures and improving bone strength.

Alendronate is commonly used in the treatment and prevention of osteoporosis, a condition characterized by weak bones.

Cholecalciferol, known as vitamin D3, is a form of vitamin D that is naturally produced in the skin when exposed to sunlight. Vitamin D is essential for the body to absorb calcium from the diet and maintain healthy bones. It plays a crucial role in regulating calcium and phosphorus levels in the body.

Fosamax Plus D is primarily indicated for the treatment of osteoporosis in postmenopausal women and is prescribed to help improve bone density and reduce the risk of fractures. The combination of alendronate and cholecalciferol in Fosamax Plus D provides both bone-strengthening effects and the necessary vitamin D supplementation.

Use and Dosage

Read the Medication Guide provided by your pharmacist before you start taking this medication and each time you get a refill. Follow the instructions very closely to make sure your body absorbs as much drug as possible and to reduce the risk of injury to your esophagus. If you have any questions, ask your doctor or pharmacist.

This medication is usually taken once per week unless otherwise directed by your doctor. Choose the day of the week that best fits your schedule and take it on that day each week.

Take this medication by mouth, after getting up for the day and before taking your first food, beverage, or other medication. Take it with a full glass (6-8 ounces or 180-240 milliliters) of plain water. Swallow the tablet whole. Do not chew or suck on it. Then stay fully upright (sitting, standing, or walking) for at least 30 minutes and do not lie down until after your first food of the day. This product works only if taken on an empty stomach. Wait at least 30 minutes (preferably 1 to 2 hours) after taking the medication before you eat or drink anything other than plain water.

Do not take this medication at bedtime or while you are still in bed. It may not be absorbed, and you may have side effects.

Other medications may decrease the absorption of this product, such as quinapril, certain forms of didanosine (chewable/dispersible buffered tablets or pediatric oral solution), sucralfate bismuth subsalicylate, mineral oil, orlistat, cholestyramine, and colestipol. Certain supplements and foods may also interfere with the absorption of this medication, such as calcium or iron supplements, vitamins with minerals, antacids, olestra, dairy products (such as milk, and yogurt), mineral water, coffee, tea, and calcium-enriched juice. Do not take these products for at least 30 minutes (preferably 1 to 2 hours) after taking this medication.

Take this medication regularly to get the most benefit from it. Remember to take it on the same day each week. It may help to mark your calendar with a reminder. Talk to your doctor about the risks and benefits of long-term use of this medication.

Side Effects

Get emergency medical help if you have any signs of an allergic reaction to Fosamax Plus D: hives; difficulty breathing; swelling of your face, lips, tongue, or throat.

Stop using Fosamax Plus D and call your doctor at once if you have:

  • Chest pain or new or worsening heartburn
  • Difficulty or pain when swallowing
  • Pain or burning under the ribs or in the back
  • New or worsening heartburn
  • Severe joint, bone, or muscle pain
  • New or unusual pain in your thigh or hip
  • Jaw pain, numbness, or swelling

Common Fosamax Plus D side effects may include:

  • Mild heartburn or stomach upset
  • Diarrhea, gas, or constipation
  • Mild joint or back pain
  • Headache

Cautions

Before taking alendronate/vitamin D3, tell your doctor or pharmacist if you are allergic to it; or to other bisphosphonates; or other vitamin D products; or if you have any other allergies. This product may contain inactive ingredients, which can cause allergic reactions or other problems. Talk to your pharmacist for more details.

Before using this medication, tell your doctor or pharmacist your medical history, especially of: disorders of the esophagus (such as esophageal stricture or achalasia), trouble swallowing, trouble standing or sitting upright for at least 30 minutes, low calcium levels, kidney problems, stomach/intestinal disorders (such as ulcers).

Some people taking alendronate/vitamin D3 may have serious jawbone problems. Your doctor should check your mouth before you start this medication. Tell your dentist that you are taking this medication before you have any dental work done. To help prevent jawbone problems, have regular dental exams, and learn how to keep your teeth and gums healthy. If you have jaw pain, tell your doctor and dentist right away.

Before having any surgery (especially dental procedures), tell your doctor and dentist about this medication and all other products you use (including prescription drugs, nonprescription drugs, and herbal products). Your doctor or dentist may tell you to stop taking alendronate before your surgery. Follow all instructions about stopping or starting this medication.

This drug is not recommended for use in children. Studies have shown that many children who took this drug had severe side effects such as vomiting, fever, and flu-like symptoms.

Tell your doctor if you are pregnant or plan to become pregnant. Alendronate may stay in your body for many years. You should not become pregnant while using this medication. Discuss the risks and benefits with your doctor.

Cholecalciferol (vitamin D3) in this medication passes into breast milk. It is unknown if alendronate passes into breast milk. Consult your doctor before breastfeeding.

FAQ

What happens if I miss a dose?

If you forget to take Fosamax Plus D on your scheduled day, take it first thing in the morning on the day after you remember the missed dose. Then return to your regular weekly schedule on your chosen dose day. Do not take two (2) tablets in one day.

What happens if I overdose?

Seek emergency medical attention. Drink a full glass of milk and call your local poison control center or emergency room right away. Do not make yourself vomit and do not lie down.

Overdose symptoms may include muscle cramps, numbness or tingling, tight muscles in your face, seizure (convulsions), irritability, and unusual thoughts or behavior.

What should I avoid?

Do not take any other medicines including vitamins, calcium, or antacids for at least 30 minutes after taking Fosamax Plus D.
