Rexulti is an antipsychotic medication used to treat major depressive disorder (MDD), schizophrenia, and agitation related to dementia caused by Alzheimer’s disease.

Brexpiprazole is a third-generation antipsychotic and partial dopamine agonist that stimulates dopaminergic and serotonergic receptors in your brain. The medication also offers antagonistic activity on certain serotonin receptors. These properties allow Rexulti to balance chemicals in your brain to relieve symptoms of MDD, schizophrenia, and agitation from dementia.

This antipsychotic medication has been found to be an effective treatment with good tolerability and minimal effects on sexual function compared to other dopamine receptor partial agonists. Those with schizophrenia and treatment resistant MDD have also reported improvements by adding Rexulti to their therapy.

Uses and Dosage

The dose of Rexulti that is prescribed will depend on the individual and condition being treated. Follow all your doctor’s instructions and read packaging labels carefully to ensure the best results during your antipsychotic treatment.

  • Swallow tablet whole
  • Take with or without food
  • For MDD, adults should start by taking 0.5 or 1mg a day and then titrate up to 2mg once daily
  • Do not take more than 3mg of Rexulti as an adjunctive MDD treatment
  • To treat schizophrenia, most adults take between 2 and 4mg once daily
  • Start with 1mg once daily for the first 4 days before increasing to 2 and then 4mg on day 8
  • Do not take more than 4mg a day to treat schizophrenia symptoms
  • For agitation associated with dementia, most adults take 2mg once a day
  • Adults should start by taking 0.5mg once daily for a week before increasing to 1 and then 2mg
  • Do not take more than 3mg a day to treat agitation
  • Ensure you drink fluids while taking Rexulti to avoid dehydration
  • Do not suddenly stop taking Rexulti
  • If you are diabetic, regularly check your blood sugar levels while taking this medication
  • Store at room temperature away from moisture and heat
  • Keep away from children and pets

Side Effects

  • Signs of a blood clot (sudden numbness or weakness, problems with vision or speech, swelling or redness in an arm or leg)

Allergic Reaction

Do not take Rexulti if you are allergic to brexpiprazole. If you develop an allergic reaction, stop taking this medication and contact your doctor immediately.

Rexulti allergy symptoms may include:

  • Hives
  • Difficulty breathing
  • Swelling of face, lips, tongue, or throat


Rexulti is not an approved treatment for older adults with psychosis related to dementia because it may increase their risk of death.

Some young people who take Rexulti may have suicidal thoughts at the beginning of treatment. Monitor your mood or symptoms and report any changes to your doctor.

Avoid driving or performing any hazardous tasks while taking Rexulti until you know how it will affect you. Brexpiprazole may cause your reactions to be impaired.

When standing from a seated or lying position, move slowly to avoid feeling dizzy and prevent falls.

In hot weather, avoid becoming overheated or dehydrated because brexpiprazole can make it harder for your body to regulate its temperature.

Brexpiprazole’s safety and efficacy in children with MDD is unclear. The medication may or may not be safe and effective in children younger than 13 years old with schizophrenia. Consult with your doctor before giving this antipsychotic medication to children.

To ensure your safety in taking Rexulti, tell your doctor if you have ever had any of the following:

  • Diabetes or high blood sugar
  • Liver disease
  • Kidney disease
  • High cholesterol or triglycerides
  • Heart problems, high or low blood pressure
  • Heart attack or stroke
  • Seizure
  • Low white blood cell counts

If taken in the last 3 months of pregnancy, brexpiprazole may cause breathing and feeding problems or withdrawal symptoms in your child. Tell your doctor immediately if you become pregnant while taking Rexulti but do not stop taking it without your doctor’s advice.

Consult with your doctor to discuss the risks of breastfeeding while taking Rexulti.

Some drugs may interact with Rexulti and cause adverse reactions. To ensure your safety, tell your doctor about all the medications you take, including over the counter medicine, vitamins, and herbal products.

Why does Rexulti cause weight gain?

Rexulti side effects vary per person, but a common symptom is weight gain. This is possibly due to other side effects such as an increase in appetite and decrease in activity because of drowsiness and fatigue. How long does it take for Rexulti to work?
