Stiolto Respimat is an inhaler used to prevent obstructions in airways and reduce attacks in those with chronic obstructive pulmonary disease (COPD).

This inhaler medication combines tiotropium bromide with olodaterol to relax muscles in your airways to improve airflow and prevent obstructions that cause bronchospasm attacks. Tiotropium inhibits muscarinic receptors while olodaterol agonizes adrenergic receptors to improve lung function.

Combining tiotropium and olodaterol makes Stiolto Respimat a significantly effective treatment for COPD that doesn’t respond as well to monotherapy of either drug. This inhaler medication improves quality of life by reducing pulmonary obstructions and attacks.

Uses and Dosage

The dose of Stiolto Respimat will vary depending on the individual and bronchial condition being treated. Follow all your doctor’s instructions and read packaging labels carefully to ensure the best results during treatment.

  • Stiolto Respimat is for oral inhalation only
  • Inhale at the same time each day
  • Insert cartridge into the Stiolto Respimat inhaler to prime the unit before first use
  • Actuate inhaler away from you until you see an aerosol cloud, then repeat three more times
  • If not used for longer than three days, actuate once to prepare the inhaler
  • If not used for longer than 21 days, actuate inhaler until a cloud is visible
  • Most adults take two inhalations once a day
  • Do not inhale more than twice within 24 hours
  • Do not change your dose or stop using the inhaler without first speaking to a doctor
  • Use rescue medicine for bronchospasm attacks, not Stiolto Respimat
  • Store at room temperature, do not allow the inhaler or cartridges to freeze

Serious Side Effects

Some more serious adverse effects may also occur when using tiotropium and olodaterol. Stop using this Stiolto Respimat inhaler and call your doctor right away if you develop any of the following symptoms:

  • Pounding heartbeats or fluttering in chest
  • Wheezing, chocking, or other breathing issues after inhalation
  • Painful or difficult urination, more frequent urination
  • Blurred or tunnel vision, eye pain, seeing halos around lights
  • High blood sugar (increased thirst, increased urination, hunger, dry mouth, fruity breath odor)
  • Low potassium levels (leg cramps, constipation, irregular heartbeats, increased thirst or urination, numbness or tingling, muscle weakness)

Allergic Reactions

Do not use Stiolto Respimat if you are allergic to olodaterol, tiotropium, or ipratropium. If you develop signs of an allergic reaction, stop using this inhaler and contact your doctor.

Stiolto Respimat allergy symptoms may include:

  • Hives
  • Difficulty breathing
  • Swelling of face, lips, tongue, or throat


Do not use Stiolto Respimat to treat asthma, the inhaler is designed only for those with COPD. Tiotropium and olodaterol may increase the risk of death or hospitalization for users with asthma, but the risk for people with COPD is unknown.

Do not get Stiolto Respimat in your eyes; if contact occurs, rinse thoroughly with water.

Stiolto Respimat is designed to prevent and reduce bronchospasm attacks and will not work fast enough to treat attacks that have already started. Only use designated rescue medicine for active attacks.

To ensure your safety when taking Stiolto Respimat, tell your doctor if you have any of the following:

  • Heart disease, high blood pressure
  • Seizures
  • Thyroid disorder
  • Diabetes
  • Glaucoma
  • Kidney disease
  • Enlarged prostate or urination problems

Do not use another inhaler at the same time as Stiolto Respimat that contains formoterol, arformoterol, indacaterol, olodaterol, salmeterol, or vilanterol.

Some other drugs may interact with tiotropium or olodaterol and cause unwanted effects. To avoid this, tell your doctor about all the medications you take, including over the counter medicine, but especially any of the following:

  • Atropine
  • Medicine for depression, anxiety, mood disorders, or mental illness
  • Cold or allergy medicine
  • Medication for Parkinson’s disease
  • Medicine for stomach problems, motion sickness, or irritable bowel syndrome
  • Medication for overactive bladder
  • Other bronchodilator medications

The effects of this inhaler medication on unborn babies are unknown. Consult with your doctor before using Stiolto Respimat if you are pregnant or plan to become pregnant.

Ask your doctor about risks before breastfeeding while using a Stiolto Respimat inhaler.

Stiolto Respimat is not an approved treatment for anyone under the age of 18.
